Example clinical interview questions for Attention Deficit Disorder patients?

I need some example questions that a psychologist would ask during a clinical interview of an ADD/ADHD patient. I do not need any information about the disorder itself....I have plenty of stuff for that. I need specific questions relative to the disorder itself. I repeat....no research information. I need example questions or example interviews with questions and answers. Possibly case studies, but I must have the questions so I can see what type of questions I need to ask and how to develop new questions that are along the same lines. The two people I am interviewing are a 21 y/o male with ADHD and an 11 y/o female with ADD.

•Is child depressed?


•Does he have anxiety problems?


•Is he hallucinating?


•Is he delusional?


•Is it a head injury?


•Is it a seizure disorder?


an clinical interview with the child is important. This interview is needed to determine the child's reality testing, his degree of maturity, his degree of verbal skills, and so on. Ask the child if he's hallucinating. Sometimes he is, but he hasn't told anyone. Ask the child if he's anxious. Some kids have tremendous fears, but have not shared them with anyone. There is a lot going on with kids that their parents are not aware of. Other important questions are:


•How did mom do during pregnancy?


•Were there any problems at all during pregnancy?


•Was there any exposure to drugs or alcohol prior to birth?


•When did he learn to walk or crawl?


•How about speech development, any problems?


•Did he have very many ear infections?


•Have there been any head injuries, high fevers, or seizures?


Head injuries and seizures can look just like attention deficit hyperactivity disorder, but aren't. They may require different treatment options.
Reply:They will llook at the DSM-4 and ask you which of these sounds like you. Then they will ask for a list of current symptoms and ask what the problems are.
