I am presently in the United States on J1 research and I need to change to J1 clinical because I got a residency position. There is a lot of requirements to be fulfilled before an approval by the department of state. What would be my best argument for this 'change of goal' by the department of state? Would I need to get a lawyer?

You don't have to explain a change of goals, you will need to say that your actual research objective is related to the future clinical activity.
